Was hoping/figuring Perfect Blue and or Requiem for a Dream would show up. I liked Requiem for a Dream a lot, though it's been ages since I've seen it and will be ages until I want to see it again. I liked Perfect Blue even more, way more actually. I heard the director bought the rights to Perfect Blue so he could borrow some scenes for Requiem for a Dream.
One thing Requiem for a Dream did have over Perfect Blue was the bathtub scene, in my opinion. Jennifer Connolly screaming under water in the fetal position was perfect. That's one of my favourite ever scenes.

It's high because of {Spoiler} how the tension is built up. I would say it's comparable to the uneasiness you get from seeing Mary Elizabeth Mastrantonio's character choke and drown in The Abyss.
You may have better scenes in other movies if you base the ranking on skin shown that's for sure.

The first scene I thought of was Edgar Kennedy in Duck Soup,who finds a surprise guest in his bathtub.
