I think they count yellow and red cards, then draw lots if the fouls are equal.

I wonder if it has ever happened. No. But in 1990, when best third place teams also qualified, Ireland and Holland had identical points and goal difference so lots were drawn to decide who officially was second and third. Holland were given third and then had to play West Germany. Wikipedia |
